What it is

actually do.
A quick intramuscular B12 shot for people with a genuine deficiency or absorption problem. Offered in Bradford, Ontario, not sold as an energy fix.
The short version
If you are deficient in B12, an injection is an effective and inexpensive way to correct it. If you are not deficient, it will not do very much for you.
That sentence is the entire honest content of this treatment, and it is deliberately at the top of this page because B12 injections are widely marketed as an energy boost for anyone. They are not that.
What B12 does and why deficiency matters
Vitamin B12, cobalamin, is required for red blood cell formation, for the normal function of the nervous system, and for DNA synthesis. Deficiency produces a recognisable set of problems: fatigue, breathlessness on exertion, pallor, a sore or smooth tongue, pins and needles or numbness in the hands and feet, poor balance, low mood and difficulty concentrating.
The neurological effects are the reason deficiency is worth taking seriously rather than shrugging at. Prolonged, untreated deficiency can cause nerve damage that does not fully reverse.
Why an injection rather than a tablet
Absorbing B12 from food is a surprisingly elaborate process. It requires stomach acid to release it from protein, a carrier called intrinsic factor made in the stomach, and a specific receptor at the far end of the small intestine to take it in. Interrupt any step and oral intake stops working, regardless of how much you swallow.
That is why the people who genuinely need injections are the people with an absorption problem: pernicious anaemia, where intrinsic factor is absent; previous gastric or small bowel surgery; coeliac disease and inflammatory bowel disease; long-term use of metformin or acid-suppressing medication; and reduced stomach acid with age.
Strict vegans and long-term vegetarians without supplementation are a different category, an intake problem rather than an absorption one, and often manageable orally.
Why we ask about blood work
Because otherwise nobody, including us, knows whether it is doing anything.
Testing before treatment tells you whether there is a deficiency to correct and gives you a baseline to measure against. It can also reveal that something else is causing your symptoms, which is far more useful than a monthly injection that quietly does nothing.
Our Nurse Practitioner can review your history and advise on what testing is worth doing, and our Naturopathic Doctor, Dr. Erica Vriend, is often involved where nutrition and absorption are part of a bigger picture.
Who this suits, and who it does not
It suits you if you have a documented deficiency, a known absorption problem, a relevant medical or surgical history, or a diet that makes deficiency likely.
It does not suit you if your levels are normal and you are hoping for an energy lift, in which case we will tell you honestly that we do not think it will help. It is not appropriate if you have a known allergy to the preparation, and it is contraindicated in Leber’s hereditary optic neuropathy.
At the appointment
Your history and any relevant results are reviewed. The injection is given into the muscle of the upper arm or the buttock by a registered nurse using a small needle. It takes moments.
Afterwards
The site may ache for a day, much like after a vaccination, and mild bruising is possible. There is no recovery period and no restriction on your day.
Your schedule is set clinically and then reviewed, rather than continuing on standing order. If your levels have corrected and the cause has been addressed, we will tell you that you can stop.
If it is wrong for you, we say so before you book again. A nurse-practitioner-led clinic has a licence at stake. That is the whole reason we are careful - and the reason the honest answer sometimes is "not this, and not yet".

Being straight with you
When we would
tell you no.
Every clinic lists who a treatment suits. This is the other half. If any of these is you, say so at the consultation and we will talk about what else might work, or about waiting.
- Your B12 level is normal and you want an energy lift. It will not give you one.
- You have Leber's hereditary optic neuropathy. B12 in this form is contraindicated for you.
- You absorb B12 normally. A tablet will do the same job for less money.
- You are allergic to cobalamin or to anything else in the preparation.
- You want injections on standing order without ever measuring a level. We will not run that.

Nothing here happens on the same visit you first walk in, unless you want it to and it is right for you. This is the order we work in.

Consult
We look at what is actually bothering you, ask what you have tried, and tell you honestly whether this is the right treatment. Costs nothing and commits you to nothing.

Plan
Your clinician maps the plan with you and agrees it before you commit to anything. The plan is signed off by our Medical Director before treatment begins.

Treatment
Your clinician carries out the plan exactly as it was agreed. You are told what is happening, where and why, before it starts - and the quoted price does not change on the chair.

Review
We see you again to check the result and adjust the plan for next time. The first appointment is the start of the plan, not the whole of it.

What is a B12 injection?
It is vitamin B12 given as an intramuscular injection, usually into the upper arm or the buttock. Because it goes into muscle rather than through the gut, it bypasses the absorption step entirely, which is the whole reason the injection exists as a treatment.
Who actually needs B12 injections?
People who cannot absorb B12 properly and people with a documented deficiency. That includes anyone with pernicious anaemia, people who have had gastric or small bowel surgery, people with coeliac or inflammatory bowel disease, long-term users of certain medications including metformin and acid-suppressing drugs, and people on strict vegan diets without adequate supplementation. Older adults absorb it less efficiently too.
Will a B12 shot give me energy?
Only if you are deficient. If your B12 level is normal, an injection is very unlikely to make you feel more energetic, and we are not going to tell you it will. Fatigue has a long list of causes and most of them are not B12. If you are tired, the useful step is finding out why.
Should I have blood work first?
In most cases yes, and we will recommend it. Knowing your actual level tells you whether an injection is treating something or simply passing through you. It also creates a baseline so you can tell whether treatment is working, which is impossible if you never measured anything to begin with.
Can I take a tablet instead?
For most people with adequate absorption, yes, and oral B12 is cheaper and easier. The injection is specifically useful when absorption is the problem, because no oral dose fixes a gut that cannot take it in. If a tablet would work for you, we will say so rather than booking you in for injections.
Does it hurt?
It is a quick intramuscular injection and most people describe a brief sting and an ache in the muscle afterwards, similar to a routine vaccination. The site can feel tender for a day. It is one of the shortest appointments we offer.
How often would I need it?
That depends entirely on why you are having it, and it is set clinically rather than by preference. Someone with pernicious anaemia has a very different schedule from someone correcting a temporary dietary shortfall. Your interval is decided at assessment and reviewed rather than left running indefinitely.
Who should not have one?
Anyone with a known allergy to cobalamin or to a component of the preparation, and anyone with Leber's hereditary optic neuropathy, where B12 in this form is contraindicated. If you have a bleeding disorder or are on anticoagulants, tell us. We will also review any other injections or supplements you are already receiving to avoid duplication.
