Filler is a structural treatment, not a wrinkle cream in a syringe
The common assumption is that filler goes into a line. Most of the time it does not. Facial ageing is largely a loss of support: fat pads descend and shrink, bone remodels, and the soft tissue that used to sit high in the cheek slides forward and down. The folds you notice at the corner of your nose or the front of your jaw are usually the visible consequence of that, not the problem itself.
So a good filler plan often treats somewhere other than the place that bothers you. Rebuilding support in the mid-face can soften a nasolabial fold better than injecting the fold directly. Projecting a recessed chin can do more for a jawline than anything done to the jawline itself.
The areas we treat
Cheeks and mid-face for support. Temples where hollowing has made the upper face look narrow. Chin and jawline for profile and definition. Lips, for volume, shape, hydration or correcting an asymmetry you have had since you were a teenager. Under-eye hollows, but only after a careful look at whether filler is genuinely the right answer there, because a lot of under-eye darkness is pigment or thin skin and filler will not touch either.
Who this suits, and who it does not
It suits you if you have identifiable volume loss or a structural feature you want changed, if you understand it is temporary, and if you are willing to build over more than one session where that is the safer route.
It does not suit you if you are looking for skin quality improvement, because that is what skinboosters, polynucleotides and resurfacing are for. It does not suit you if you have significant skin laxity and no volume loss, because adding weight to lax skin is not a lift. And we will not treat you if you have an active infection in the area, if you are pregnant or breastfeeding, or if what you are asking for would put a vessel at risk for a cosmetic gain we consider marginal.
At the appointment
You will be assessed sitting upright with a mirror, and we will usually ask what you dislike before we tell you what we see. The area is cleaned, topical anaesthetic is applied where appropriate, and most of the products we use also contain lidocaine.
Your injector will work with either a needle or a blunt cannula depending on the area and the risk profile. You may be asked to sit up and check the result partway through. Nothing is finished in one continuous run without you seeing it.
Afterwards
Expect swelling, and expect it to be worse than the final result. Lips swell substantially. Bruising is common and more likely if you have taken anti-inflammatories, fish oil or alcohol beforehand. You will be given aftercare that covers heat, exercise, dental work and sleeping position.
We ask you to come back for a review before you decide the treatment is finished. Judging filler at day three is judging swelling, not the outcome you paid for.
If it is wrong for you, we say so before you book again. A nurse-practitioner-led clinic has a licence at stake. That is the whole reason we are careful - and the reason the honest answer sometimes is "not this, and not yet".

Every clinic lists who a treatment suits. This is the other half. If any of these is you, say so at the consultation and we will talk about what else might work, or about waiting.
- You are pregnant or breastfeeding. We will see you once you have finished.
- Your problem is skin quality rather than lost support. Skinboosters or polynucleotides suit you better.
- You have laxity with no volume loss. Adding weight to lax skin is not a lift.
- You have an active infection or cold sore in the treatment area.
- You have an event in the next fortnight. Lips and eyes swell unpredictably.
- You want more than your anatomy safely allows. We will decline to overfill an area.

Nothing here happens on the same visit you first walk in, unless you want it to and it is right for you. This is the order we work in.

Consult
We look at what is actually bothering you, ask what you have tried, and tell you honestly whether this is the right treatment. Costs nothing and commits you to nothing.

Plan
Your clinician maps the plan with you and agrees it before you commit to anything. The plan is signed off by our Medical Director before treatment begins.

Treatment
Your clinician carries out the plan exactly as it was agreed. You are told what is happening, where and why, before it starts - and the quoted price does not change on the chair.

Review
We see you again to check the result and adjust the plan for next time. The first appointment is the start of the plan, not the whole of it.

Questions people actually ask

What are dermal fillers made of?
The fillers we use are hyaluronic acid gels, a sugar molecule your body already makes and can break down. They are cross-linked to different degrees, which is what makes one product firm enough to project a chin and another soft enough to sit under thin lip skin. Because they are hyaluronic acid, they can be dissolved with hyaluronidase if something needs correcting.
Can filler be dissolved if I do not like it?
Yes, hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase. That is one of the main reasons we work with hyaluronic acid rather than permanent products. Dissolving is its own appointment with its own assessment, and it removes your natural hyaluronic acid in the area too, so it is a correction tool rather than an undo button.
How much filler will I need?
That is decided by assessment, not by a number you pick in advance. Some faces need one syringe placed well; some need staged treatment across more than one visit. We quote per syringe and per area once we have looked at you, and if we think you need less than you came in asking for, we will say so.
Will it look obvious?
Not if the plan matches your anatomy. Filler looks obvious when it is placed in the wrong plane, when too much is used in one sitting, or when the same shape is applied to every face. We treat the structure that is actually deficient rather than chasing a look, and we will decline to overfill an area even if you ask.
Is filler safe?
It is a medical procedure with real risks, and the serious one is vascular occlusion, where product enters or compresses a blood vessel. It is rare, and it is why who injects you matters more than which brand they use. Our injectors are Registered Practical Nurses working to a plan signed off by our Nurse Practitioner, and we keep hyaluronidase on site. Does filler stretch your skin?
There is no good evidence that appropriately dosed hyaluronic acid filler permanently stretches skin. What does happen is that repeated overfilling, year after year, without reassessing the face, changes proportions in a way people do not like. We reassess at every visit rather than simply repeating the last treatment.
How is this different from Botox?
Filler adds volume and support; a neuromodulator relaxes muscle. They solve different problems and often work best together, with the neuromodulator softening movement lines and the filler restoring the structure underneath. If you only need one of them, we will tell you which one.
Can I have filler if I have an event coming up?
Book it well before the event, not the week of. Swelling and bruising are normal and unpredictable in timing, particularly around the lips and eyes, and the final shape settles after the swelling goes. We would rather move your appointment than have you photographed mid-settle.
